Annual Membership -$999.99 billed annually
For this plan, you get unlimited access and can book as many appointments as you want. This also includes access to call provider anytime and request medication refills
Pay as You Go Patients
$68 dollars per 15 minutes visit
$35 for follow up visits
$25.00 dollars for medication refills/request
$25 dollars for additional time in increments of 15 minutes
$15 charge if appointments not cancelled within 2 hours of appointment time.
$20 charge for no shows
Family Monthly Plan
$ 99.99 per family member - minimum of 3 family members
